After all required documentation is received, Fr. Michael Machacek will be in touch with you to go over the date/details, etc.

REQUESTING A COPY OF A BAPTISMAL CERTIFICATE:
Please email [email protected] to request a copy of your Baptismal Certificate.
Please include the following information:
* Provide the name as it appeared on the original Certificate
* Date of Birth
* Mother's maiden name
* Name
* Reason you are requesting a copy
You will be notified via email when the certificate is ready for pick-up. Please note there is a $10.00 fee.
